Select the front polygon and Bevel using a Height of 2 and an Outline Amount of -2. Then Inset it 1.75 units.
Next Extrude the result 13 units. Bevel it using a Height of 10 and an Outline Amount of -0.32. Bevel it once more, using a Height of 2 and an Outline Amount of -0.32.
Select the "back" polygon. Bevel it using a Height of 5 units and an Outline Amount of -2.5 units.
